Assessment and Care Planning

Our lead consultant has 20 years experience of conducting care needs assessments and writing support proposals for providers being contracted by councils and NHS. Assessments are fully documented, recognizing the expectations of the Care Act and National Institute of Care Excellence (NICE) guidance, the individual’s care needs and the screening of risks. We can assist with the construction of support proposals where required

Our consultants have long experience in the development and production of person-centred care plans that best represents the person’s care needs and utilises the input of the person, their families/advocates, external professionals and any other stakeholders. The aim should be for the person to be as independent as their potential allows and creates strategy/opportunity for that person to further develop their skills, independence levels and quality of life. We believe people thrive best when there is opportunity to recognize, reward and celebrate frequent small steps of progress.
